A clinical record done properly protects two people: the doctor, who can prove what he did, and the patient, who knows what they said yes to and what was done to them. Here there are nine risks, one per page, with the rules, the rulings and the penalties.

Someone who was not informed can claim damages even if everything went well.

A gap weighs against whoever wrote it, but on its own it is not enough.

It is mandatory, and it also covers events from ten years earlier.
05Photographs and data
To publish a before and after you need a separate consent.
06Healthcare advertising
Offers, discounts and promotions are prohibited.
07Product traceability
You must be able to say which lot you used on which patient.
08Off-label use
It can be done in individual cases, and it must be justified in the medical record.
